Dr. Leendert van Zyl is a South African born physician who has been practicing family medicine in Barrie, Ontario since 2003. He is a member of the Barrie Family Health Organization and Barrie Family Health Team.

Dr. van Zyl's practice ranges from prenatal and well-baby care to end-of-life support. He works with excellent nursing staff that help manage office flow and take bookings for various nursing procedures. His office and staff are shared with two other physicians and a nurse practitioner. His administrative staff and office manager are very diligent in managing bookings, specialist appointments, and various other patient needs.

Together, Dr. van Zyl and his team work hard to serve the patients they have developed close relationships with over the years. Dr. van Zyl continues to develop his practice with the patient's needs in mind. This website has been developed by Dr. van Zyl to better serve and support patients by providing practical medical and wellness information.

Dr. van Zyl earned his MBChB from University of Pretoria in 1997. In 2002 and 2003, he received his LMCC and CCFP, respectively.

What should I do if I have a problem after hours?

We are part of the Barrie Family Health Organization/Team and we operate after hours at two locations and a tele clinic:

What do I do if I run out of my medication?

Please contact your pharmacy and ask them to fax a request for medication renewal to our office. Allow 3-5 days for response.

How do I get a driver's medical done?

You can book a driver’s medical with our office. This service is not covered by OHIP and our fee is $125. This can be paid by debit, cash or e-transfer. A driver's medical involves a medical history, physical exam with vision test, urine test, and completing the driver’s medical form. Please come prepared to give a urine sample, remember your glasses if you need them for driving and bring the appropriate documentation. If you do home blood pressure readings, please bring them with you. Completed forms are given back to you to present to the MTO.

How do I get forms filled out by my doctor?

When you need forms completed, please first carefully review the forms to familiarise yourself with the information required and that you fit the criteria mentioned in the form. Sign the consent to release medical information. Some governmental forms can be billed to OHIP. Most other forms are not covered and may be a cost to you, or may be billable to the requesting party. Please review dates required on the form, like last day worked, days missed etc. and have these readily available. Contact the office and inform the receptionist that you have forms that you would like to be completed. Some forms can be done without a visit, some can be done in conjunction with a telephone consultation and some will require you to book an office visit. Please allow time for processing forms and try not leaving them to be done at the last minute.

What should I do if I have been referred to a specialist?

Referrals are sent directly from our office to the specialist physician. You do not need to contact them.

Most specialist offices will contact you directly with an appointment date or instructions. Some may contact us and we will then notify you. If you have not received any communication within 4 weeks (or 2 weeks if more urgent), please inquire with our office.
